The body length of the four-eyed butterfly is 7.5cm. The body of the adult fish is tall and oval, and the surface above the head is straight. The snout is pointed, but does not extend into a tubular shape, and the anterior nares have nasal valves. The edge of the front operculum is finely serrated, the operculum membrane and the isthmus are connected, and the teeth of the two jaws are densely arranged with fine tips. The body is covered with medium-sized scales, and the lateral line rises steeply to below the 9th-10th spine of the dorsal fin and then drops below the base edge of the dorsal fin. The dorsal fin is simple. The body of the four-eyed butterfly is generally gray-blue, with yellow on the head, abdomen, anal fin and lower side of the body. The upper and lower parts of the body each have 17-20 gray-black diagonal stripes extending to the end of the dorsal fin and the end of the anal fin, which meet at the center of the body side to form an arrow-shaped pattern pointing toward the head.
